二、參加競談的單位（公司）要求：
（一）一般資格條件
1、具有有效的獨(dú)立法人資格。
2、具有良好的商業(yè)信譽(yù)和近三年內(nèi)沒有被政府部門通報(bào)的違規(guī)違紀(jì)行為。
3、具有履行合同所必須的專業(yè)技術(shù)能力，具備行業(yè)相關(guān)資質(zhì)，及代理產(chǎn)品授權(quán)書。
4、有依法繳納稅收和社會(huì)保障資金的良好記錄。
5、提供以下資料：企業(yè)營業(yè)執(zhí)照、組織機(jī)構(gòu)代碼證、稅務(wù)登記證、相關(guān)資質(zhì)證書、業(yè)績表、法定代表人授權(quán)書、承諾書，以上復(fù)印件需加蓋公司鮮章（原件備查）。
6、密封報(bào)價(jià)，加蓋公司鮮章。

（二）特殊資格條件
1、參加競談的單位（公司）須為產(chǎn)品的生產(chǎn)商或代理商，若為代理商，須提供具有產(chǎn)品合法代理資格的證明文件；
2、產(chǎn)品若屬醫(yī)療器械，應(yīng)具有醫(yī)療器械經(jīng)營企業(yè)許可證；
3、產(chǎn)品若屬醫(yī)療器械，提供有效期內(nèi)的《醫(yī)療器械注冊證》以及與之配套的《醫(yī)療器械產(chǎn)品生產(chǎn)制造認(rèn)可表》或《醫(yī)療器械注冊登記表》。
